I make this affidavit to induce the Town of Southold to issue a certificate 'of pre-existing use for said premises . • From, on or about April 10, 1964 until on or about October 20, 1977 I was one of_ the fee owners of the said premises . Further- more, at the time that I purchased the said premises on or about April 10, 1964, I was advised by the former owners as- to the facts surrounding the prior status of the premises and, especially, its relationship to zoning in the Town of Southold.- Accordingly, I state of my own personal knowledge and belief that the said premises as same are depicted upon the attached survey have existed in such fashion without any additions or altera- tions whatsoever since prior to April of 1957 . This application must be filled in typewriter OR ink, and submitted in ' a to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Zulewski & Wf. Occupied: Richard & Joan Tandy � 1 Upon receipt of an application for a Pre-Certificate of Occupancy, I made an inspection of buildings at this location. I was admitted to dwelling by Mrs . Tandy and began this inspection at approximately 4:00 P.M. The dwelling is a two story, wood framed structure constructed _o-f' a cement block foundation. First floor has a kitchen,toilet room, living room with fireplace ,. sun room, dining room and family room. Second floor contains three bedrooms and a full bathroom. Heat is furnished to all rooms from an oil fired hot water furnace located in full cellar. Accessory building:,is a wood framed one and one-half car garage constructed on a poured concrete foundation. The following violations of the Housing Code , Chapter 52, Code of the Town of Southold, N.Y. were found: Gas cooking range : no shut off valve at unit. Article V, Section 52-52 B.2 Electrical: service panel located in furnace room, front panel cover removed. Article V, Section 52-56 A.1 . Accessory building : stair, (exterior) on rear of building leading to attic storage area have 6th & 8th steps that are broken. Article II, Section 52-27 A. . Article III, Section 52-30 A.
