This 1 assessment is intended to supplement other information used by a Town of Southold agency in making a determination of consistency. *Except minor exempt actions including Building Permits and other ministerial permits not located within the Coastal Erosion Hazard Area. Before answering the questions in Section C, the preparer of this form should review the exempt minor action list, policies and explanations of each policy contained in the Town of Southold Local Waterfront Revitalization Program. The existing 5'-5"X 4'-0" (21.68 sq.ft.) covered entry porch on the north side of the house was replaced with a new 13'-3"X 5'-9" (76.1 sq.ft.) covered entry porch. 2. The secondary entry door on the north side of the house and the existing 3'-3"X4'-7" (14.88 sq.ft.) entry porch were eliminated. 3. A new 5'-0"X5'-1-1/2" (25.6 sq.ft.) outdoor shower was added on the north west corner of the house. 4. The existing 20'-3-'/2"X5'-0" (101 .45 sq.ft.) covered wood deck on the south side of the living room was replaced with a new 33'-9-°/2" x 9'-6" (321 .0 sq.ft.) wood deck with no roof. 5. The existing 26'-3-Y2"X9'-6" (249.75 sq.ft.) screened porch on the south side of the house was re-built to include new decking, screening and framing in the ceiling to support a new 26'-3"X9'-4" (319.17 sq.ft.) 2nd floor deck above. 6. Twenty four windows and 2 exterior doors were replaced as per window schedule. 7. The first floor powder room 4'-5"X 7'-Y ( 32 sq.ft.) was renovated. 8. A new 4'-0"X 4'-5" (17.68 sq.ft.) laundry closet was added on the first floor. 9. The kitchen and dining room on the first floor (25'-11-'/2"X 18'-4-'/2"" 476.8 sq.ft.) was renovated. 10. Approx 6'-0" of 4'-0" tall fencing was added to the propane tanks at the north east corner of the house. 11 . On the second floor, bedrooms # 2 & # 3, ( 25'-9-'/2"X18'-1" 466.28 sq.ft.) were combined by eliminating the existing roof, dormers and kneewalls and adding new 8'-0" walls and new roof framing to create one bedroom with bathroom and walk in closet. 12. On the second floor, bedrooms # 4 & #5 (24'-8"X 28'-1" 692.73 sq.ft.) were altered by eliminating the existing roof, dormers and kneewalls and adding 8'-0" walls and new roof framing. 13. On the second floor Bathroom # 3 ( 6'-2"X9'-11" 61 sq.ft.) was renovated and a shower was added. 14. On the exterior of the house new cedar shingle siding was added. 15. New gutters and leaders were added to contain storm water run off in proposed drywells as noted on survey. 16. A new IA sanitary system is proposed to be placed on the landward side of the house. 17. A new poured concrete foundation will be added in the footprint of the existing foundation to raise the house from the current fin fl. elevation of 7.55' to an elev. of 10.42' which will bring the structure into compliance with current FEMA regulations.
