This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,streets,and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses,or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines,streets,building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied,the Building Inspector shall state the reasons therefor in writing to the applicant.

ALL that certain plot,piece or parcel of land, situate, lying and being at New Suffolk, in the
Town of Southold, County of Suffolk and State of New York,known and designated as Lot No. 1 as
shown on a certain map entitled, "Map of Thomas McCarthy", and filed in the Office of the Clerk of
the County of Suffolk on June 15, 1994 as Map No. 9520,which said lot is more particularly
bounded and described as follows:
BEGINNING at the corner formed by the intersection of the northerly side of King Street
with the easterly side of Fourth Street;
RUNNING THENCE North 00 degrees 08 minutes 30 seconds East along the easterly side of
Fourth Street, 45.64 feet to the division line between Lots 1 and 2 on the above mentioned map;

Dear Sir/Madam:
I am,writing in connection with the enclosed Pre Existing Certificate of Occupancy dated
October 10, 1990, issued by Gary J. Fish. As you will note,this Certificate covers four(4) one
family dwellings. Subsequent to the issuance of the Pre Existing Certificate of Occupancy the
parcel was divided into four(4) lots. Enclosed is a description for the lot in question, lot#1,tax
map lot 8.1. Also enclosed is a copy of the filed map and the tax map showing lot 8.1. Further
enclosed is a survey dated February 14, 1995, showing the house at the lot above described
which is at the corner of Front and King Street in New Suffolk.
As discussed with Mr. Fish, we represent the owner of that lot, lot#1 on the map and tax map lot
8.1, and respectfully request that a single Pre Existing Use Occupancy be issued for that lot.
I am also enclosed a Pre Existing Certificate of Occupancy application together with a check in
the sum of $100.00.
I would appreciate your telephoning me if there are any questions and also advising when the Pre
Existing Certificate of Occupancy is issued for the dwelling at this lot.
