Y. 11971 Board of Trustees; I hereby apply for a Permit to replace the 'fill on our residential property located in Mattituck , N.Y. , on Peconic Bay Blvd . This need for the replacement' of the fill is due to the erosion of approximately 70 to 75 yards of existing fill during the."ptorm of December 11 and 12, 1992.- The exact .location is as described on the attached -...-.-.-Copy of the Deed for-the '-subject property. ---As stated on the - . Deed , .the property is .bounded by Peconic Bay on the South, - and Peconic Bay Blvd . - on ,the North . Also attached are copies of four photographs taken in early January, 1993, showing the extent of the erosion due to the December storm., Attached also are two sketches, with approximate dimensions, showing the various specific areas of the storm erosion, and the proposed replacement of the eroded fill . The amount to be replaced will be the same amount as that lost during the storm, thus restoring the property to its original condition and configuration as before the damage - and erosion caused by the storm of December 11 and 12, 1992. The work will be performed by Ray Nine Sand and Gravel . I have also filed , this date, with the NYS Dept . of Environmental Conservation, for approval to have this storm damage and erosion repaired and replaced . I trust that this information is sufficient for the issuance of the required permit to get this work accomplished .
