COUNTY OF SUFFOLK) AGNES LINDSAY, residing at 830 Bayview Avenue, Mattituck, New York 11952, being duly sworn, deposes and says: I was the owner of premises located on Main Road, Mattituck, New York, Suffolk County Tax Map #1000-122.00-03.00-006.000, as shown on the enclosed survey, until July 24, 1987 at which time it was sold to Harvey Bagshaw. The main building on the premises was built prior to 1957 and was originally used as a theater. The exterior has not been changed so as to require a building permit. The other two buildings on the premises have Certificates of Occupancy, specifically #Z4706 and Z8043. To my knowledge and belief, no other alterations were made to the premises that would require a Certificate of Occupancy.
