�' µr --.The validity of this permit is or may be subject to the approval of other governmental or municipal
authorities. The Town accepts no responsibility in applying for or obtaining such approval. In the
event that such approval is necessary, the holder of this permit shall not commence operations here-
under until such approval has been obtained in writing. The failure to obtain such other approval when
required shall subject this permit to immediate revocation by the Trustees upon receipt by the
Trustees• of written notice from such other governmental or municipal authorities of its refusal or
��� disapproval. _
The applicant does by the acceptance of this permit, assume all responsibility for operations under-
taken pursuant to this permit, and shall take all precautions for the prevention of injuries to persons
and property resulting from such operations. By such acceptance, the applicant also agrees to indemni-
fy and save harmless the Town, and its officers, agents and.employees from any and all claims arising
from operations under this permit and any and all acts or omissions of applicant, his agent and
employees.
The applicant and the owner and occupants of the premises upon which the operations authorized by
this permit are being conducted, do, by the acceptance of this permit, give consent to the Town, and
its officers and employees to enter upon the premises where such operations are being conducted to
make such inspections as the Town may deem necessary to insure that such operations are being con-
ducted in conformity with this permit.
This operation will not substantially:
A. Adversely affect the wetlands of the town.
B. Cause damage from erosion, turbidity or siltation.
C. Cause saltwater intrusion into the fresh water resources of the town.
D. Adversely affect fish, shellfish or other beneficial marine organisms, aquatic wildlife and vege-
tation or the natural habitat thereof.
E. Increase the danger of flood and storm-tide damage.
F. Adversely affect navigation on tidal waters or the tidal flow of the tidal waters of the town.
G. Change the course of any channel or the natural movement or flow of any waters.
H. Weaken or undermine the lateral support of other lands in the vicinity.
I. Otherwise adversely,affect the health, safety and general welfare of the people of the town.

reverse side, provide-a SCALE DRAWING which will show Ordinary High Water Mark as well

portions which extend offshore from the O. H. W. M. State Adjacent property
owners.

A. This application is to be completely filled in by typewriter or ink and submitted to the Town
Clerk in qucdruolicete, accompanied by writter- permission from the owner of the property if 'not
the same as the applicant.
B. Pilot Plan must be dre•.vn to scale on the reverse side of this cpplicction showing the Icccti:n
of property, the names and lacctions of owners of adjoining properties, and a detciled description
of proposed project. (Note: Ccpies of the Pilot Plan �ney be mcce by a copy machine and atr=hed

C. No operation shall be initiated by the applicant until oil permits that ere required are issued.
D. A detailed statement of the rehc'--iIitction and prLposed condition of the premises after .the
work is completed must be given, including a survey if required.
• E. If permit is being sought for a more sizeable excavation Than the construction of a private
single deck or jetty, then this application must be accompanied by a survey and topaarephical mop
certified by a registered land survey or Professional Engineer. The horizontal control of survey
shell be based upon an approval local coordinate system. The vertical control for elevation and sound-
ings shall be based upon United States Coast and Geodetic Survey and/or United States Geologic;.)

F. A statement must cccompany the application describing any known prior operations conducted _
on the premises in question and whether any prior licenses or permits have been issued to erect
structures or to dredge or deposit fill on said premises and whether any such permits or licenses
were ever revoked or suspended by a governmental agency.

NEGATIVE DECLARATION
NOTICE OF NO SIGNIFICANT EFFECT ON THE ENVIRONMENT
Dated: September 28, 1984
Pursuant to Article 8 of the Environmental Conservation Law
State Environmental 'Quality Review and 6NYCRR Part 617, Section
617. 10 and Chapter 44 of the Code of the Town of Southold, notice
is hereby given that the Southold Town Trustees, as lead agency
for the action described below, has. determined that the project,
which is unlisted will not have a significant effect on the
environment.
DESCRIPTION OF ACTION
Application of Bernard 0. Monson for a Wetland Permit to construct a
bulkhead, platform, ramp and float and to dredge and backfill on
property located at 750 Budd' s Pond Road, Willow Point, Southold.
The project has been determined not to have a significant

action described below, has determined that the project, which is

DESCRIPTION OF ACTION
Application of Bernard O. Monson for a Wetland Permit to construct

PROJECT DESCRIPTION:
This request is to apply for a permit to construct a bulkhead and
docking assembly, and to dredge in an unnamed canal off. Budd' s Pond, Shelter
Island Sound at Southhold, Suffolk County, New York for the purpose of
erosion control and providing mooring for our private recreational boat.
The proposal is to construct 70 Linear feet of timber bulkhead
connecting to existing bulkhead at the northerly end (lot #15). At the
southerly end connect to existing bulkheading of adjacent lot (#13). The
bulkhead will be constructed generally along the Mean High Water.
The docking assembly would consist of 4 foot by 4 foot platform, a
4 foot by 14 foot ramp and a 6 foot by 20 foot floating dock adjacent to the
proposed bulkhead.
It is also proposed to dredge an area waterward of the proposed
bulkhead with continued maintenance of such dredged area.
The area, to be dredged to a depth of 4 feet below Mean Low Water
is approximately 15 feet wide and would extend the length of the bulkhead.
Approximately 170 cubic yards of sand and silt material would be dredged
initially and placed as backfill behind the proposed bulkhead. It is
estimated that maintenance dredging would be necessary every 3 to 5 years
and that each dredging could generate approximately 60 cubic yards of
material which would be disposed of in the town landfill .

Gentlemen:
Per the attached letter of August 25, 1985, you were kind
enough to extend my original permit for an additional six
months to construct bulkheading at my new home.
Unfortunately, due to weather conditions and contractor
schedules, this work has been delayed until now and I will
be unable to have the work completed by the permit expira-
tion date of April 25, 1986.
I have received an extension of my DEC permit until December
31 , 1986 and my Army Corps of Engineers permit does not
expire until February 8, 1988.
I would appreciate whatever action is necessary to obtain an
additional time extension to my town permit number 183. The
work should be completed sometime in May so as not to inter-
fere with my neighbors or the summer boating season. Thus,
an extension for several months should be adequate.
