Pinkham North Fork Country club Main Road Cutchogue NY 11935 Re: SCTM #¢109-4-7.1 Dear Mr. Pinkham, The following action was taken by the Board of Town Trustees during its regular meeting held on September 30, 1993 regarding the above matter: WHEREAS, NORTH FORK COUNTRY CLUB applied to the Southold Town Trustees for a permit under the provisions of the Wetland Ordinance of the Town of Southold, application dated December 10, 1991 and, WHEREAS, said application was referred to the Southold Town Conservation Advisory Council for their findings and recommendations, and, WHEREAS, a public hearing was held by the Town Trustees with respect to said application on Thursday, September. 30, 1993 at which time all interested persons were given an opportunity to be heard, and, WHEREAS, the Board members have. personally viewed and are familiar with the premises in question and the surrounding area, and, WHEREAS, the Board has considered all the testimony and documentation submitted concerning this application, and, WHEREAS, the Board has determined that the project as proposed will not affect the health, safety and general welfare of the people of the town, NOW THEREFORE BE IT, t RESOLVED that the Board of Trustees approves the application of NORTH FORK COUNTRY CLUB to construct the 16th tee and other operations as described in the enclosed project description and survey dated as revised August 26, 1993, subject to, the runoff be contained and native vegetation buffer areas be maintained and the 15th fee area B not to be disturbed at all on bank. Present at the meeting were Chairman John Holzapfel, Bruce Loucka, John Hagerty, and Pat Isaksen. Moved by John Holzapfel, seconded by Bruce Loucka it was RESOLVED to recommend to the Southold Town Trustees APPROVAL of the Wetland Application of CLEAVES POINT VILLAGE 38-7-4.2 & 8 and 38.2-1-45 existing dock modification. MUNIER 128-6-1 which is a Grandfather Permit for stairs and decks for bulkheads; deck and bathhouse approximately 12' x 40' located in between bulkheads. 4030 Peconic Bay Blvd., Laurel Vote of Council: Ayes:. All Motion carried. Moved by Patricia Isaksen, seconded by John Holzapfel, it was RESOLVED to recommend to the Southold Town Trustees APPROVAL of the Wetland Application of VICTOR CATALANO 128-6-13.3 to replace (within 18") 103 I.f. of timber bulkhead. Wall to be backfilled with 350 c.y. of clean backfill to be trucked in from upland source and delivered over owner's property. , Remove an existing concrete stairway and replace with a 5' x 7' wood stairway. Property damaged by winter storms. The Council recommends approval and suggests the applicant may wish to tie in the adjacent bulkheads to create a solid line. Peconic Bay Boulevard, Laurel. Vote of Council: Ayes: All Motion carried. Moved by John Holpzalfel, seconded by Patricia Isaksen, it was RESOLVED to recommend to the Southold Town Trustees DISAPPROVAL of the Wetland Application of ROBERT TAYLOR 126-6-9.1 to construct 58' of timber retaining wall no more that 2' in front of existing retaining wall, with a 30' return on the east property line. Approximately 40 c.y. of clean backfill to be trucked in in from upland source. The Council recommends disapproval as submitted because there is plenty of room behind the existing seaward bulkhead (not retaining wall); the proposed plan would be 2' in front of adjacent western bulkhead which would create an angle, which can create erosion problems. The Council also questions the MHW, it does not appear to be correct on the submitted plans. Plantings shall be installed in Area B as shown on approved plans except at the 15th Tee and there shall be no disturbance to vegetation or topography seaward of the bank at the 15th Tee. 4. There shall be no pesticide, herbicide, irrigation or fertilizer application in the maintained rough of the 16th Tee, 5. There will no draining nor directing of surface runoff toward tidal wetlands from the new tee, greens and fairway. The golf cart paths may only be constructed of crushed stone or equivalent pervious surface materials. 7. Hay bales shall be installed at seaward toe of disturbed areas for the 16th Tee, trap, fairway construction and remain in place until such time that disturbed areas are vegetated. 8. All Supplemental plantings in the Area A and B shall be maintained at an 85% survival rate for a minimum of five Years from completion of the project. 9. There shall be no alteration or disturbance to vegetated tidal wetlands. 10. All debris shall be removed to an approved solid waste management facility. 11. Any debris or excess material from construction of this project shall be completely removed from the adjacent area (upland) and removed to an approved upland area for , disposal. No debris is permitted in tidal wetlands. i2 . The storage of construction equipment and materials shall be confined to within the project work site and/or upland areas greater than 75 linear feet distant from the tidal wetland boundary. 13. If seeding or soil stabilization is impracticable due to the time of year, a temporary mulch shall be applied and final seeding shall be performed at the earliest opportunity when weather conditions favor germination and growth, but not more than six months after project completion. 14 . Prior to the ultimate expiration of this permit the permittee will cause all fill and exposed faces to be vegetated to prevent erosion. 15. All disturbed areas where soil is temporarily exposed or stockpiled for longer than one month shall be stabilized in accordance with temporary seeding or ground cover measures as stated in the attached Erosion Control Sheet. To protect the values of the tidal wetland, a permanent vegetated buffer zone shall be established. There shall be no disturbance to the natural vegetation or topography within an area extending 150 linear feet landward of the tidal wetland boundary. 17. All fill shall consist of "clean" gravel and soil (not asphalt, slag, flyash, broken concrete or demolition debris) . Supplementary Special Conditions (A) thru (F) attached. A copy of this permit, including all conditions and approved plane, shall be available at the project site whenever authorized work is in progress. The permit sign enclosed with the permit shall be protected from the weather and posted in a conspicuous location at the work site until all authorized work has been completed. B. The permittee 'shall require that any contractor, project engineer, or other person responsible for the overall supervision of this project reads, understands, and complies with this permit and all its general, special, and supplementary special conditions. Any failure to comply precisely with all of the terms and conditions of this permit, unless authorized in writing, shall be treated as a violation of the Environmental Conservation Law. If any of the permit conditions are unclear, the permittee shall contact the Division of Regulatory Affairs at the address on page one or telephone (516) 444-0365. C. If project design modifications become necessary after permit issuance, the permittee shall submit the appropriate plan changes for approval by the Regional Permit Administrator prior to undertaking any such modifications. The permittee is advised that substantial modification may require submission of a new application for permit. D. At least 48 hours prior to commencement of the project, the permittee and contractor shall sign and return the top portion of the enclosed notification form certifying that they are fully aware of and understand all terms and conditions of this permit. Within 30 days of completion of the permitted work, the bottom portion of that form shall also be signed and returned, along with photographs of the completed work and, if required, a survey. E. For projects involving activities to be undertaken in phases over a period of more than one year, the permittee shall notify the Regional Permit Administrator in writing at least 48 hours prior to recommencing work in subsequent years. F. The granting of this permit does not relieve the permittee of the responsibility of obtaining a grant, easement, or other necessary approval from the Division of Land Utilization, office of General Services; Tower Building, Empire State Plaza, Albany, NY 12242 (516) 474-2195, which may be required for any encroachment upon State-owned lands underwater. The permittee must submit a separate written application to the Department for renetial , m)dification or transfer of this permit. All organic debris will be removed from the site, none will be buried or burned. Trees specifically marked on the plan will be retained . REGRADING: The existing contours of the land will be retained as much as possible except for the drainage control and contouring around fairway and greenside traps . Bredemeyer, III President, Board of Trustees JMB:j t NORTH FORK COUNTRY C LU B GENERAL PROJECT DESCRIPTION Create new 16th teeing ground and fairway in wooded area to the east of existing 15th green and 16th fairway; clear trees as required; regrade as required to prevent runoff into adjacent wetlands; install several new sand traps; seed or sod new fairway, near adjacent "rough" areas, and teeing area; install extension of existing irrigation system as required. Extend existing back ("blue") teeing area of 15th hole, using fill from regrading of new 16th hole; seed or sod the new area as required; install irrigation system extension as required. All operations will be conducted using the methods and materials generally recognized as suitable and environmentally safe in golf course construction and management. It must be noted that while the narrative descriptions and drawings are as accurate as possible, it is possible that during construction situations may develop which would require minor changes to the present written application. It is re- quested that sufficient flexibility be granted by the regula- tory agencies to permit minor changes which are obviously within the spirit and intent of this application, which do not impose environmental burdens, and which do not extend beyond the proposed seaward limits of the project as submitted. Detailed descriptions of the proposed operations are attached. NORTH FORK COUNTRY CLUB WORK DESCRIPTION FOR NEW 16TH HOLE A new tee and fairway for the 16th hole of the North Fork Country Club will be constructed in accordance with the enclosed plans and specifications contained herein . The existing green will be retained undisturbed except for modification of surrounding traps and associated mounds. All work will be performed in 'accordance with this work description . CLEARINGS Trees will be cut down , stumps removed, and brush cleared as required for the playing area as shown on the plan . All organic debris will be removed from the site, none will be buried or burned. Trees specifically marked on the plan will be retained. REGRADINGe The existing contours of the land will be retained as much as possible except for the drainage control and contouring around fairway and greenside traps. Where it is necessary to significantly alter contours top soil will be stripped and replaced after regrading . Collection ponds and berms will be created as shown on the plans to prevent runoff into the surrounding wetlands. TEES AND TRAPSv Teeing areas and mounds around traps will be constructed by bringing in fill as required . After final shaping and placement of top soil , clean sand will be brought in and placed in depressions in the mounds to create fairway and greenside sand traps. IRRIGATION SYSTEMS A two line irrigation system will be installed which will cover the area shown on the plan. Any runoff from the irrigation system will be contained by the drainage control described above. SEEDINGi In preparation for seeding the fairway and near rough areas the area will be plowed and disced. "Nitroform" fertilizer and lime will be applied prior to seeding . Certain limited areas may be sodded instead of seeded . If during maturation of the turf it is determined that an insecticide is required "Banol" will be used at the rate of 2 lbs. per 1000 sq . ft. If during the maturation of the turf it is determined that some form of weed control is required a herbicide will be applied . as required to control the specific variety of weed.
