Deponent makes this affidavit for DORIS D. REEVE, who is the owner of the premises located at (No #) North Road, Mattituck, New York, Suffolk County Tax Map #1000-140..00-02 .00-013 . 000, as shown on the enclosed survey. The property was conveyed to Doris D. Reeve on March 15, 1933 by Mary D. Reeve. The residence and garage were built prior to 1957. To my knowledge and belief, no changes or alterations were made to the premises that would require a Certificate of Occupancy, except for the addition which was added in 1966, for which a Certificate of Occupancy was issued. I make this affidavit to the Town of Southold for a Certificate of Pre-Existing Use or the residence and garage.
